Generally, it would be an agency doing a bait and switch, not an independent escort (as with them word of mouth is their pipeline to bookings), so they have a driver. The chick shows up, is nothing like her pics or the description given over the phone and you don't want her to stay. You pay the kill fee which is generally what the driver charges for shipping the whores.

Like say you book a hooker from BackPage @$250 for an hour, outcall (meaning she comes to you). $50 to the driver, $100 to the agency and $100 to the whore. Then they will try to upsell you from there. "That money is just for my time, if you want me to do anything you have to tip."
I don't know if they have to give the agency part of the tips or not. If they have a flat out pimp, I think he gets everything, then doles out an allowance.
